Consent for Treatment and Product Use

  • I have completed any Consultation Form accurately.
  • I have been candid in revealing any conditions or medications that could prohibit treatment(s) or product use, such as pregnancy, cold sores, use of hormones, recent treatments, and allergies.
  • I acknowledge that there is always a possibility of an adverse reaction with any facial, chemical peel, microdermabrasion, microneedling, waxing, products sold, used in treatment, and dispensed (online or in store), and that is the case regardless of precautions taken. I accept full responsibility for any adverse reactions that occur from my negligence and I agree to not hold the service provider or Skin and Acne Specialist LLC (Rachel’s Skin Suite) responsible for any adverse reactions.
  • I hereby authorize the practitioner to perform the treatment(s) we’ve discussed.
  • I verify that all information disclosed here is true and correct.
  • I will hereby immediately contact Skin and Acne Specialist, LLC to consult over any adverse reactions or questions I might have after my treatment. Contact 920-210-0370.

Cancellation Policies

  1.  Late Cancel Fee – For cancellations or rescheduling under 24 hours notice, there is a $50 reschedule fee. This fee is credited on your account for the next visit, this fee covers lost time, daycare, fuel, and/or travel time.
    • 24 Hour notice is required for cancellations.
    • You may cancel by calling, emailing: [email protected], or online (if before 24 hours notice).
    • Your appointment has been reserved for you and time is valuable. In order to honor my time and the time of others, please kindly give notice appropriately.
    • A “No Show” results in a $50 Fee  or a forfeit of one service in a package or gift certificate. The fee must be paid before any more services are rendered. The fee is nonrefundable.
    • Late arrivals: The treatment time will be adjusted to fit the time frame specially set aside for you.
    • Please arrive 5 minutes before your scheduled time frame.

Return Policies

  1. Return Policy:
    • Refunds or exchanges may be given on unopened & unused products if notified within 10 days of purchase. Product must be returned within 30 days in resale condition.
    • Customer is responsible for return shipping.
    • Shipping charges are non refundable at the time of purchase.
    • All sales are final on services, treatments, pre-purchased services, used or open products, or gift certificates. Packages and Gift Certificates: Please kindly use gift certificates within two years.

  1. Flat $6.95 Shipping for Orders $99.99 or less.
  2. FREE Shipping on Orders $100 or more.
  3. We ship to the Continental United States only via USPS.

To schedule a service:

  1. Go to the Service you would like and click on the picture.
  2. Once the page opens up go to the Blue BOOK NOW Button. Book Now
  3.  Click on BOOK NOW!
  4.  You will now go to Schedulicity! Click on Schedule Now!
  5. Pick your service!
  6. Pick your Provider.
  7. Pick your Day/Time!
  8. Now Schedule!

 

I want to buy a gift certificate for Services!

Answer:

To buy a gift certificate for Services:

  1. Click on the Services or Shop Page!
  2. Scroll down to the bottom.
  3. Choose Gift Certificate for Services!
  4. Choose an amount!
  5. Add to cart!
  6. View Cart (Black box toward the right side of the screen!)
  7. Proceed to Check out!
  8. Fill in your information and Pay Pal to complete your check out!
  9. Print out your gift Certificate!
  10. Bring your gift certificate in when you schedule your service!

I want to buy a gift certificate for Products!

Answer:

To buy a gift certificate for Products:

  1. Click on the Services or Shop Page!

  4. Choose an amount!
  5. Add to cart!
  6. View Cart (Black box toward the right side of the screen!)
  7. Proceed to Check out!
  8. Fill in your information and Pay Pal to complete your check out!
  9. Print out your gift Certificate and keep your code!
  10. Online ordering: When you are ready to shop, choose your items, check out and put in that code to redeem your Product Gift Certificate! 
  11. In Store: Bring your gift certificate when you stop in!

I want to buy a gift certificate or PRE-PAY for the EXACT Service Listed!

Answer:

To buy a gift certificate or pre-pay for a specific Service(s).

  1. Find the Services Page!
  2. Scroll down to the service.
  3. Choose the service by clicking on the Black shopping Cart Button or Click on the picture and click on the Blue Add to Cart Button. 
  4. Add to Cart.
  5. View Cart (Black box toward the right side of the screen!)
  6. If you have a gift Certificate you can apply the code you printed out.)
  7. Proceed to Check out!
  8. Fill in your information and Pay Pal to complete your check out!
  9. Print out your information!
  10. You will still need to schedule your service through Schedulicity. Your service is NOT scheduled! See instructions below to schedule your service!
  11. To schedule a service:
