Consent for Treatment and Product Use

  • I have completed any Consultation Form accurately.
  • I have been candid in revealing any conditions or medications that could prohibit treatment(s) or product use, such as pregnancy, cold sores, use of hormones, recent treatments, and allergies.
  • I acknowledge that there is always a possibility of an adverse reaction with any facial, chemical peel, microdermabrasion, microneedling, lash lift, lash and brow tint, Sculplla, waxing, products sold, used in treatment, and dispensed (online or in store), and that is the case regardless of precautions taken. I accept full responsibility for any adverse reactions that occur from my negligence and I agree to not hold the service provider or Skin and Acne Specialist LLC and The Washington County School of Esthetics responsible for any adverse reactions.
  • I hereby authorize the practitioner to perform the treatment(s) we’ve discussed. By clicking the book now and scheduling, you are acknowledging that an inherent risk of exposure to COVID-19 exists in any public place where people are present, by scheduling a service, you and any guests voluntarily assume all risks related to exposure to COVID-19 and agree not to hold Skin and Acne Specialist LLC dba The Washington County School of Esthetics; or any of their employees, customers, contractors, or students liable for any illness or injury.

Cancellation Policies

Late Cancel Fee – For cancellations or rescheduling under 24 hours notice, there is a $50 reschedule fee.

  • 24 Hour notice is required for cancellations.
  • You may cancel by calling, emailing: [email protected], or online (if before 24 hours notice).
  • Your appointment has been reserved for you and time is valuable. In order to honor my time and the time of others, please kindly give notice appropriately.
  • A “No Show” results in a $50 Fee or a forfeit of one service in a package or gift certificate. The fee must be paid before any more services are rendered. The fee is nonrefundable.
  • Late arrivals: The treatment time will be adjusted to fit the time frame specially set aside for you.
  • Please arrive 5 minutes before your scheduled time frame.

School Cancellation Policies

Due to Covid 19 we will ask you to:

1. Please wear a mask at all times in the main areas. We will remove your mask in the treatment room during the service.

2. Please Sanitize hands before entering!

3. Please leave as much items in your car as possible. Bring only what’s necessary, keys, etc.

4. Please leave family at home.

5. Please wait in your car until the time of your appointment or a couple minutes before. (We have more cleaning to do so we ask that you wait in your vehicle if you come early.)

DO NOT COME IF YOU ARE ILL, please contact us to reschedule we will not charge any cancellation fees if notice is given 2 hours before scheduled service arrival time. We want to maintain a safe and healthy environment!

  • Refunds or exchanges may be given on unopened & unused products if notified within 10 days of purchase. Product must be returned within 30 days in resale condition.

I accept full responsibility for any adverse reactions that occur from my negligence and I agree to not hold the student service provider at The Washington County School of Esthetics or Skin and Acne Specialist LLC responsible for any adverse reactions.
  • I hereby authorize the student practitioner to perform the treatment(s) we’ve discussed. I understand that the conditions listed above may be contraindications to our treatments. If any of the above items apply to me, and I elect to receive a facial or body treatment service, I accept the risks associated, I release The Washington County School of Esthetics, and all persons associated with The Washington County School of Esthetics and Skin and Acne Specialist LLC, from any and all liability.   • I understand that all services rendered at The Washington County School of Esthetics are performed by students. These students are under the supervision of licensed Instructors. I also understand that the services I have requested are ordinarily harmless to normal skin, hair or nails, but many cause damage due to their current condition. I am aware that if I have a challenge with the results of my service, I must notify the school within 5 business days of the student service. The Washington County School of Esthetics recommends changing into a gown for all services, and is not responsible to any damage to clothing. Any gratuity for the students must be in cash or check!
